4.6 At this point the who, what, where, why, and
when of the near-miss is understood, and the human errors, structural/machinery/equipment/outfitting
problems, and external factors that led to the near-miss, have been
identified. The next step is to better understand the causal factors
that contributed to the near-miss. There are a variety of identification
methods for this purpose, including taxonomies of causes. These can
be used for deep probing past the most evident causes.
