11 Test Report

 The test report should include the following information:

  • .1 name and address of the test laboratory;

  • .2 date and identification number of the test report;

  • .3 name and address of client;

  • .4 purpose of the test;

  • .5 method of sampling;

  • .6 name and address of manufacturer or supplier of the product;

  • .7 name or other identification marks of the product;

  • .8 description of the tested product;

  • .9 date of supply of the product;

  • .10 date of test;

  • .11 test method;

  • .12 identification of the test equipment and used instruments;

  • .13 conclusions;

  • .14 deviations from the test method, if any;

  • .15 test results including observations during and after the test; and

  • .16 date and signature.
