3.1.1 It is essential
that Surveyors are aware of the contents of current LR Safety Notices
and they should satisfy themselves that adequate precautions are taken
to ensure safety during surveys. In many countries, regulations or
standards exist for staging and entry into tanks. These are not always
observed and therefore Surveyors are urged to assure themselves that
hazards have been reduced to a minimum before surveys are undertaken.
3.1.2 Surveyors should
fully recognize the dangers associated with entering any space with
limited openings and/or unfavourable ventilation, e.g. an enclosed
space. Authorization to enter any such space must always be obtained
from the ship’s nominated responsible person who, prior to sanctioning
entry, should have established the potential hazards and have made
appropriate arrangements. These should include testing of the atmosphere
within the space for oxygen depletion or enrichment and flammable
and/or toxic substances, ensuring there is adequate illumination and
continuous ventilation, communications, and rescue/resuscitation equipment
as necessary.
3.1.3 Cases have
occurred of serious accidents due to collapse or inadequacy of staging.
Detailed regulations for the safe construction and maintenance of
gangways, staging and ladders are in force in some countries, but
unless they are properly enforced, the risk of an accident cannot
be eliminated or minimized. Surveyors should satisfy themselves that
staging and ladders are safe before they use them. Careful attention
must be given to the contents of Safety Notices and any Safety Manuals
available, and where necessary hazardous areas should be promptly
brought to the notice of responsible management.
