Section 6 Reporting

6.1 General

6.1.1 As far as possible, the First Entry reporting of Hull Classification Surveys should be carried out in accordance with established procedures.

6.2 The report C11(N) – Appendix 3

6.2.1 This report is the record of Watertight and Weathertight Closing Arrangements and should be prepared in accordance with the sample report C11(N) which is based on a deck by deck division of the ship, see Ch 22 Appendix 3 Form C11(N).

6.2.2 The report consists of a number of pro formas:

  1. Front page and amendments.

  2. Doorways.

  3. Hatches (including escape scuttles).

  4. Ventilators.

  5. Air and sounding pipes.

  6. Scuppers, inlets and discharges, including:

    1. Flood drains.

    2. Salvage and overboard discharge ports.

  7. Sidelight windows and viewing ports.

  8. Protection of the crew.

  9. Freeing ports.

  10. Magazine blow out plates.

  11. Side shell openings including visors and ramps.

  12. Other special features (aircraft lifts/ramps).

  13. Back page (for Renewal Survey Endorsement Records).

6.2.3 The report should be prepared with a single front page and a single back page and completed on a deck basis or deck/zone basis for larger ships, selecting and using as many sections/pages as are necessary for that deck or zone from the list above.

6.2.4 The information given on any section should refer to the deck underfoot and the adjacent bulkheads. Overhead openings are detailed in the sections dealing with the deck above.

6.2.5 On completion, the report should be signed and dated by the designated Surveyor, and reviewed and signed by the Authorising Surveyor.

6.2.6 A copy of the C11(N) is to be placed on board for the use of the attending Surveyors and the information of the Ship’s staff.

6.2.7 Minor amendments to the record can be made by adding additional pages, and noting that this has been done on the front page. Larger changes will involve changing pages.
